A reflective intake for new clients. Take your time, this form is the beginning of the work.

Before you begin

New to this? Please don't feel shy or intimidated. I work with clients across the full spectrum of experience, from those exploring their desires for the first time to seasoned practitioners. Curiosity is enough to begin.

This form is longer and more intimate than a typical intake, intentionally so. The questions are designed to help you reflect on what draws you, and to give me the texture I need to design a session that actually resonates with who you are.

There are no wrong answers. Leave items blank if they don't apply. Take your time. Most clients find the act of completing this form useful in itself, long before we ever meet.

A note on what makes a submission I can actually respond to: real verification details (your real name, your real employer or provider profile, placeholders won't pass), and answers that show you've engaged with the questions. Brief is fine. Empty or filler tells me you aren't ready, and I won't reply. Same-day requests are difficult if you haven't been verified before, feel free to ask, sometimes I can make it happen, but it's usually smoother to get verified in advance so you can make a last-minute ask later.
